Maintenance & Airworthiness Report

Please use this module to submit report which may contain any condition, object or activity with the potential of causing injuries to personnel, damage to equipment or structures, loss of material, or reduction of ability to perform a prescribed function in Maintenance activity such as:

1. Aircraft Damage (FOD, hangar/ ramp equipment, towing, etc.)
2. Aircraft Deterioration (burning, corrosion, contamination, crack, etc.)
3. Compliance – Documentation (AD/ SB, IPC, MEL, ect.)
4. Compliance – Overrun (planned maintenance, life control, etc.)
5. Engineer left cabin floor hatch open
6. Lack of Ground Engineer
7. Parts lost during Flight
8. Significant Process Issue
9. Significant Spares Issue, Significant Technical Issue
10. Task Error and etc.
