Flight Safety Report

Please use this module to submit report which may contain any condition, object or activity with the potential of causing injuries to personnel, damage to equipment or structures, loss of material, or reduction of ability to perform a prescribed function in Flight Operation such as:

1. Aircraft Document
2. Airport Mgmt (Airside) (airbridge, FOD, marking, surface, etc.)
3. Air Traffic Service (communication, navaid, near collision, etc.)
4. Configuration Warnings
5. Crew Schedule
6. Diversion (medical, closed airport, other)
7. Doors (operations, condition, etc.)
8. Emergency Declaration
9. Environment (animal strike, volcanic ash, smoke, laser, etc.)
10. Flight Management (collision, unstable approach, fuel management, crew rest, etc.)
11. Go Around/ Missed Approach/ Overshoot
12. Human Performance Problem (fatigue, work schedule, etc)
13. Overspeed/exceedance , etc.
