Cargo Operation Report

Please use this module to submit report which may contain any condition, object or activity with the potential of causing injuries to personnel, damage to equipment or structures, loss of material, or reduction of ability to perform a prescribed function in Cargo Operation such as:

1. Cargo Damage
2. Missing Cargo
3. Missing Document
4. Undelivered Cargo
5. Mislabelled
6. Improper packaging
7. Improper Handling (storing, offload, etc.)
8. Facility (lighting, safety equipment, storage location, etc.).
